• When collecting the information for this, the data that I will capture will be your email address, physical address, date of birth, telephone number, GP details and next of kin. These details will be stored securely on a password-protected laptop and kept separate from your session notes. Your details will be anonymised and password-protected. To be compliant with my insurers I have to keep these details for 5 years and then be able to destroy them once the time has elapsed.
